Incident reporting is essential for protecting children, ensuring safety, and meeting Arizona state licensing requirements. This training will teach you how to recognize when an incident must be reported and when it must be escalated as an “unusual incident” under Arizona Title 21 (R21-7-207). You will learn how to document events accurately, identify situations requiring immediate hotline reporting, and follow required timelines for notifying supervisors, placing entities, and the Department of Child Safety. Understanding these expectations ensures compliance with state regulations and supports the well-being of every child in care.
1. Identify Reportable Incidents
- Distinguish between incident reporting and Title 21 “unusual incidents.”
- Recognize situations requiring emergency services, crisis response, or external reporting.
2. Apply Title 21 Reporting Requirements
- Identify which events require immediate notification to the DCS Centralized Intake Hotline according to R21-7-207(C).
- Determine which incidents require follow-up notifications within 2 hours, 24 hours, or 48 hours, based on subsections (D), (E), and (H).
3. Complete an Accurate and Objective Incident Report
- Document the who, what, where, when, and how of an incident using factual, unbiased language.
- Write a chronological narrative that meets both organizational and state documentation standards.
4. Demonstrate Proper Notification Procedures
- Identify who must be notified during various types of incidents (supervisor, placing entity, guardians, crisis response, law enforcement, DCS).
- Understand escalation procedures when an incident involves suspected abuse, runaway situations, medical emergencies, or critical safety risks.
